Summary: The current review issue provides an update on selected and difficult topics across the clinical spectrum of dermatopathology, in the hope of providing the reader with an understanding of recent advances in the field and the diagnostic tools to tackle these challenges. The diagnosis of melanocytic tumors continues to represent one of the most challenging areas in surgical pathology, and Spitzoid tumors in particular remain a problematic and contentious topic. Significant progress has been made over the past years in our understanding of the underlying genetics and the behavior of these distinctive melanocytic tumors, which is discussed here in detail. Molecular advances have also had a particular impact on mesenchymal tumor pathology. The current body of knowledge as it relates to cutaneous mesenchymal tumors is reviewed with an emphasis on the development of novel markers. This is complemented by additional overviews on atypical fibroxanthoma, cutaneous malignant nerve sheath tumor, and vascular tumors with spindle cell morphology. Updates on skin adnexal tumor pathology include a review of the morphological spectrum of sebaceous neoplasms, a discussion of diagnostically challenging malignant sweat gland tumors, and Merkel cell carcinoma. The topic of cutaneous lymphoproliferative disorders is focused on the spectrum of tumors with a cytotoxic phenotype and Epstein-Barr virus-associated disorders. This is contrasted with an overview of reactive and inflammatory conditions mimicking lymphoproliferative disease, the so-called pseudolymphomas. Two articles dealing with inflammatory skin disorders conclude this review: an overview of the histologic spectrum of connective tissue disease in the skin and an outline of the significance of immunofluorescence in autoimmunone bullous disorders.
